During your first appointment you will need to provide your medical and surgical history, any medications you are currently taking and any allergies to medicines. Tests that have been completed since the date your headaches first started will be very helpful (both the report and, if an MRI or CT scan was done, then a copy of the images too if possible). Finally, please provide a list of any treatments that you have tried and whether they were effective or not and any side effects you experienced.
Before your first visit, please complete our intake forms and bring them in with you or come at least 15 minutes before your appointment time to fill these out. You can download the forms from the Patient Forms section below.
Blue Cross Blue Shield PPO, Humana PPO and Medicare are the only insurance plans that are currently accepted. However, we will bill your insurance even if we are not a part of your insurance network. You may be responsible for a greater part of the bill if you are not a part of these accepted networks. If you do not have medical insurance and are paying cash, you will receive a discount but will be required to pay at the time of the visit.
A valid credit card authorization will be required at the time the appointment is made to hold the appointment. There is a cancellation fee of $125 if you cannot keep your appointment, and have not informed us at least 24 hours or 1 business day prior to the scheduled time. Please note, we are not open on Wednesdays.
